Thank you so much for attending an FMMC concert! Please use the form to submit your information so that we may contact you in the event of a positive case connected to one of our concerts. All of our musicians are vaccinated and we require masks and distancing at our events, which will mitigate the vast majority of risk involved, but we still want to ensure we have a way to contact you if a positive case arises.
If you learn that you or someone you have associated with has tested positive, and that that positive test coincided with your attendance at an FMMC concert, please email manage@fmmc.org as soon as possible.
